This short self-assessment can help determine whether you may benefit from an Emotional Support Animal and qualify for an ESA letter.

You’ll receive your results after entering your email, and they’ll also be sent to our office if you choose to schedule an ESA evaluation.

Instructions: Over the past two weeks, how often have these felt true for you?
(0 = Not at all • 1 = A few days • 2 = More than half the days • 3 = Nearly every day)

I’ve felt anxious, restless, or on edge
I’ve had trouble sleeping or sleeping too much.
I’ve felt emotionally drained or overwhelmed.
I’ve lost motivation or interest in things I used to enjoy.
I’ve struggled to concentrate or stay focused.
I’ve experienced trauma or loss that still affects me.
I’ve felt lonely, unsupported, or disconnected from others.
I’ve experienced physical symptoms (like headaches or fatigue) linked to emotional stress.
I’ve withdrawn from social activities or relationships.
I often feel unsafe or emotionally unsettled in my environment.
I believe a pet could bring comfort, structure, or grounding to my daily life.
I feel that an emotional support animal would positively impact my mental health and help me feel more stable.
